b. RATIFICATION OF AN INDEPENDENT CONTRACTOR AGREEMENT AND HOLD HARMLESS INDEMNIFICATION AND EMERGENCY INFORMATION/CONSENT FORM WITH KENNETH B. ACKERMANN      (D)  (C)  
 

Ratification of an Independent Contractor Agreement and Hold Harmless Indemnification and Emergency Information/Consent Form with Kenneth B. Ackermann to provide miscellaneous graphic design projects for the schools and district.

         
c. APPROVAL OF COMPREHENSIVE SCHOOL SAFETY PLAN      (D)  (C)  
 

Each school is required by SB 187 to annually review and approve a Comprehensive Safe Plan. The Safe School Plan addresses areas such as physical environment, school climate, safety strategies, child abuse reporting procedures and numerous others. The Safe School Plan also provides information on Standardized Emergency Management, Initial Response to Emergencies, Immediate Response Actions and Emergency Procedures.

         
d. APPROVAL OF AN OVERNIGHT EXTENDED FIELD TRIP BY OAK MIDDLE SCHOOL STUDENTS TO MONTEREY, CA, MARCH 26-29, 2015, TO ATTEND THE NEXT GENERATION JAZZ FESTIVAL      (D)  (C)  
 

The Oak Middle School Jazz Band will have an opportunity to compete in the 2015 Monterey Next Generation Jazz Festival.  Students will be competing with middle school jazz band ensembles from other school.  While at the festival students will perform for judges and audience members as well as attend workshops and watch competing schools.

         
e. APPROVAL OF OVERNIGHT EXTENDED FIELD TRIP BY LOS ALAMITOS HIGH SCHOOL WRESTLING TEAM TO BAKERSFIELD, CA, MARCH 5 - 7, 2015, TO ATTEND THE CIF STATE WRESTLING CHAMPIONSHIPS      (D)  (C)  
 

Two Los Alamitos High School students qualified for the CIF Wrestling State Championships.

         
f. RATIFICATION OF AN ADDENDUM TO AN INDIVIDUAL CONTRACT WITH APPROACH LEARNING AND ASSESSMENT CENTERS, INC.      (D)  (C)  
 

Ratification is requested of an addendum to an individual contract with Approach Learning and Assessment Centers, Inc. to provide individual speech session for at student with special needs (Student #1190115418).

         
g. ADOPTION OF 2015-2016 BUDGET CALENDAR      (D)  (C)  
 

Pursuant to Board Policy No. 815, the Board is to approve an annual budget calendar no later than the first regular Board meeting in March of each year.

         
h. APPROVAL OF MEMORANDUM OF UNDERSTANDING BETWEEN LOS ALAMITOS UNIFIED SCHOOL DISTRICT AND THE LOS ALAMITOS EDUCATION FOUNDATION (LAEF) FOR 2015-16      (D)  (C)  
 

The Los Alamitos Education Foundation (LAEF) will offer enrichment courses for a fee for 2015-16 throughout the District.

         
i. APPROVAL OF RENEWAL OF SERVICE AGREEMENT WITH SCHOOLDUDE.COM, INC. FOR THE USE OF WEB-BASED WORK ORDER SOFTWARE TO BE USED DISTRICTWIDE      (D)  (C)  
 

This agreement provides a web-based work order system which is currently utilized Districtwide.

         
j. APPROVAL OF CONTRACT WITH POST ALARM SYSTEMS FOR THE INSTALLATION OF AN INTRUSION ALARM SYSTEM AT THE MAIN DISTRICT OFFICE DURING MODERNIZATION      (D)  (C)  
 

Per the District Office Modernization and as part of ongoing security for the Los Alamitos Unified School District Office, security system installation and monitoring is required.

         
k. APPROVAL OF INDEPENDENT CONTRACTOR AGREEMENT WITH CARLOS BANUELOS TO PROVIDE DJ SERVICES FOR THE OAK MIDDLE SCHOOL SPRING DANCE      (D)  (C)  
 

Approval is requested of an independent contractor agreement with Carlos Banuelos to provide DJ services for the Oak Middle School Spring Dance.

         
l. RATIFICATION OF CONTRACT WITH RCB AND SONS, INC. FOR LANDSCAPE IMPROVEMENTS AT MCGAUGH ELEMENTARY SCHOOL      (D)  (C)  
 

Ratification is requested of a contract with RCB and Sons, Inc. for landscape improvements at McGaugh Elementary School.

         
m. RATIFICATION OF AN EVENT SPONSORSHIP AGREEMENT BETWEEN MICROSOFT AND THE LOS ALAMITOS UNIFIED SCHOOL DISTRICT      (D)  (C)  
 

Ratification is requested of an Event Sponsorship Agreement between Microsoft and the Los Alamitos Unified School District.

         
n. APPROVAL OF A LEASE AGREEMENT BETWEEN KONICA MINOLTA AND GE CAPITAL AND THE LOS ALAMITOS UNIFIED SCHOOL DISTRICT TO LEASE A COPY MACHINE FOR ROSSMOOR ELEMENTARY SCHOOL      (D)  (C)  
 

Approval is requested of a lease agreement between Konica Minolta and GE Capital and the District to lease a new copier for Rossmoor Elementary.

         
o. AUTHORIZATION TO PURCHASE CONVECTION OVEN UTILIZING THE LOS ANGELES UNIFIED SCHOOL DISTRICT PIGGYBACK BID NO. C-1059 FOOD SERVICES EQUIPMENT, CONTRACT NO. 1150100      (D)  (C)  
 

The administration is requesting and recommending use of this piggyback bid in order to secure a convection oven for the District’s food services menuing needs.

Approval is requested of an agreement with Downey Civic Theatre for a Los Alamitos High School Student Choreography Dance Show.

         
r. APPROVAL OF VISITATION AGREEMENT BETWEEN THE LOS ALAMITOS UNIFIED SCHOOL DISTRICT AND THE MANHATTAN BEACH UNIFIED SCHOOL DISTRICT      (D)  (C)  
 

Approval is requested of a Visitation Agreement between the Los Alamitos Unified School District and the Manhattan Beach Unified School District.

         
s. APPROVAL OF CONTRACT WITH POST ALARM SYSTEMS FOR THE INSTALLATION OF AN INTRUSION ALARM SYSTEM AT THE AUXILIARY OFFICE DURING MODERNIZATION      (D)  (C)

a. PRESENTATION AND APPROVAL OF 2014-2015 SECOND INTERIM REPORT AND ASSOCIATED BUDGET REVISIONS, AND CERTIFICATION OF FINANCIAL CONDITION      (D)  (V)  
 

Education Code 42131 requires the governing board of each school district to provide the County Office of Education (on or before March 15) an interim report.  The Superintendent recommends that the Board of Education approve the 2014-2015 Second Interim, Certification of Financial Condition, and associated budget revisions as presented by Patricia L. Meyer, Deputy Superintendent.

         
b. APPROVAL OF WARRANTS      (D)  (V)  
 

The Superintendent recommends that the Board of Education approve warrants totaling $1,397,670.93 for the period of February 18, 2015 through March 3, 2015.

         
c. ACCEPTANCE OF DONATIONS      (D)  (V)  
 

The Superintendent recommends that donations to the District be accepted, and that letters of appreciation be written to the donors.
